Malcolm Eye Geometry Floating Outside of Socket

I've used the Malcolm rig for a few animation projects now and this problem has happened before. This time though i forget how to solve it. Any advice on how to get the eyes to sit back in the correct spot, or how to avoid this from happening in the first place?

As far as I know there's no way to move the eye geo besides the eye aim, so I don't know how it gets like this in the first place. It seems like it happens when I have and then reopen a scene after some time.

P.S. How do you correctly embed images? Do they have to be a certain size?

I don't think I've seen that before and I'm not sure what would cause that.
